New Client Information
Thank you for choosing Lee County Veterinary Care! If you are a new client, we would appreciate it if you could take the time to fill out the New Client Registration and New Patient Registration forms and bring them to your first visit. If you are an existing client and have a new pet, filling out the New Patient Registration form before its first visit will help to speed up the check-in process. Here are some more things you should know before your first visit:
Thanks again for choosing Lee County Veterinary Care! We look forward to providing exceptional veterinary care for your animals.
- Call or stop in to create an account and set up your first appointment
- Try to show up a few minutes early so we can process paperwork and health records.
- Be sure to check out our Puppy Packages and Kitten Packages to save on the cost of the vaccine and deworming series.
- Small animals: Please bring copies of any prior health and vaccination records for your pets (from other veterinarians, breeder, prior owners, etc.). Records can also be faxed to us at 319-837-6512 or emailed to us at [email protected].
- Large animals: Our indoor working facilities are in the south side of our main building. It works the best to enter through the north driveway if you have a trailer. Please let us know before you back your trailer up to the gate so that we can make sure all of the gates, etc. are set up the way they need to be.
- Horses: Our stocks are in the garage part of the clinic. Please park your trailer out of the way of other vehicles and come into the office to let us know you are there before you get your horse(s) out of the trailer. We can either work on horses in the stocks or tied to the trailer. Castrations will be done in the yard on the north side of the clinic.
- Farm calls: We offer on-farm large animal services and we have a mobile chute. Please let us know if we will need our chute, try to let us know about everything you are going to want us to work on while we are there, and give us accurate directions to where we need to go.
Thanks again for choosing Lee County Veterinary Care! We look forward to providing exceptional veterinary care for your animals.
